چکیده

Cardiac resynchronization therapy (CRT) is a relatively new therapy for patients with symptomatic heart failure resulting from systolic dysfunction. CRT is achieved by simultaneously pacing both the left and right ventricles. Biventricular pacing resynchronizes the timing of global left ventricular depolarization and improves mechanical contractility and mitral regurgitation. Published clinical trials have demonstrated that CRT results in improved clinical status and lower mortality rate when selected patients with systolic ventricular dysfunction and heart failure are treated with CRT. This advisory identifies appropriate candidates for CRT on the basis of the inclusion criteria and results from the published clinical trials. (Circulation. 2005;111:2146-2150.)

Prinicipal component analysis of myocardial strains to optimize cardiac resynchronization therapy patient selection

Background Cardiac resynchronization therapy (CRT) is effective for selected heart failure (HF) patients but improved methods for identification of mechanical dyssynchrony are needed. Cine displacement encoding with stimulated echoes (DENSE) MRI can be used to obtain high quality strain data for LV dyssynchrony quantification. An ideal dyssynchrony parameter should be time-independent and requi...

Why QRS Duration Should Be Replaced by Better Measures of Electrical Activation to Improve Patient Selection for Cardiac Resynchronization Therapy

Cardiac resynchronization therapy (CRT) is a well-known treatment modality for patients with a reduced left ventricular ejection fraction accompanied by a ventricular conduction delay. However, a large proportion of patients does not benefit from this therapy. Better patient selection may importantly reduce the number of non-responders. Here, we review the strengths and weaknesses of the electr...

Echocardiographic Predictors of Response to Cardiac Resynchronization Therapy in 2016: Can Quantitative Global Parameters Succeed Where Segmental Parameters of Dyssynchrony Have Fallen Short?

From almost the time of the initial randomized controlled trials demonstrating a benefit to cardiac resynchronization therapy (CRT) in the early 2000s, clinicians, investigators, and industry have all been drawn to the concept that quantitative parameters derived from echocardiographic images might be a powerful tool to enhance patient selection. The need for a tool was magnified by the early o...

QRS width and mechanical dyssynchrony for selection of patients for cardiac resynchronization therapy: one can't do without the other.

Cardiac resynchronization therapy (CRT) in patients with therapy-resistant, symptomatic heart failure in the setting of reduced left ventricular (LV) ejection fraction and wide QRS complex provides symptom relief, induces reverse remodeling, and improves survival (1–3). Because approximately 30% of patients fulfilling standard selection criteria for CRT do not experience such benefits, there ha...

Cardiac Resynchronization Therapy for Heart Failure

The weight of evidence supporting the routine use of cardiac resynchronization therapy, or atrial-synchronized biventricular pacing, as a treatment for patients with moderate-to-severe chronic systolic heart failure and ventricular dyssynchrony is now quite substantial. Results from mechanistic studies, observational evaluations, and randomized, controlled trials have consistently demonstrated ...
